Who We Are
Founded in 2025 by registered nurse Chad Mangum, Ascend Health is the product of over 20 years of hands-on experience in home health, hospice, and post-acute care. Chad has owned and operated healthcare agencies across the country—from Nebraska to Alaska—and brings that same leadership and accountability to every aspect of Ascend Health today.
What We Do
Our mission is simple: to deliver a seamless continuum of care that helps patients stay out of the hospital and thrive at home. We do this through:
- Our own Home Health, Hospice, and Palliative Care services
- Close coordination with Skilled Nursing and Assisted Living Facilities
- Integrated support services including:
- Provider Services
- Durable Medical Equipment (DME)
- Transportation
